ABSTRACT

Objective:To investigate the resting state functional connectivity changes of the " triple network model" composed of salient network (SN), executive control network (ECN) and default mode network (DMN) in patients with acute mild traumatic brain injury (mTBI).Methods:From August 2020 to December 2021, forty-five acute mTBI patients (mTBI group) and 40 healthy controls (HC group) with matched sex, age, and education were included.The Montreal cognitive assessment (MoCA) scale was used to evaluate the cognitive status of all subjects.The resting state network (RNS) was established based on independent component analysis (ICA), and the SN, ECN and DMN were extracted, then functional network connectivity (FNC) was analyzed.Subsequently, the correlation between functional connectivity abnormalities and the performance of cognitive impairment was analyzed.SPSS 19.0 was used for statistical analysis and double sample t test was used for comparison between the tow groups. Results:Compared with HC group, mTBI group had enhanced functional connectivity between SN(L-insula) (MNI: x, y, z=-36, 15, 0, t=3.693)and ECN (left superior parietal gyrus, L-SPG) (MNI: x, y, z=-33, -69, 54, t=3.333)(FDR adjust, P<0.05), and decreased functional connectivity between DMN(left superior frontal gyrus, L-SFG) (MNI: x, y, z=-30, 30, 42, t=-4.063)and DMN(L-angular gyrus)(MNI: x, y, z=-21, -66, 33, t=-4.101)(FDR adjust, P<0.05). For FNC analysis, functional network connectivity in SN(IC26)-DMN(IC8) was enhanced in the acute mTBI group and decreased between SN(IC26)-DMN(IC12) and ECN(IC3)-DMN(IC12). The changes of left superior parietal gyrus functional connection were negatively correlated with MoCA score ( r=-0.627, P<0.01), and SN (IC26) -DMN(IC12) connection was positively correlated with MoCA score ( r=0.411, P=0.005). Conclusions:In patients with acute mTBI, the resting functional connectivity changes within and between the networks of the " triple network model" composed of SN, ECN and DMN, and is related to the decline of cognitive function.This will help to better understand the neuropathological mechanism of acute mTBI and post-traumatic cognitive impairment, and may become an effective imaging marker for identifying and predicting cognitive impairment after mTBI.

ABSTRACT

ObjectiveTo investigate the effect of combination of proprioceptive neuromuscular facilitation (PNF) rope training and repetitive transcranial magnetic stimulation (rTMS) on upper limb motor function of stroke patients with hemiplegia. MethodsFrom March, 2021 to February, 2022, 90 stroke inpatients with hemiplegia in the Department of Rehabilitation Medicine, the Second Affiliated Hospital of Guangxi Medical University were divided into control group (n = 30), rope group (n = 30) and combination group (n = 30) randomly. All the groups received routine rehabilitation, while the control group received routine PNF training, the rope group received PNF rope training, and the combination group received PNF rope training and rTMS, for four weeks. They were assessed with Functional Test for the Hemiplegic Upper Extremity-Hong Kong version (FTHUE-HK), Fugl-Meyer Assessment-Upper Extremities (FMA-UE) and modified Barthel Index (MBI) before and after treatment. ResultsThe scores of FTHUE-HK, FMA-UE and MBI increased in all the three groups after treatment (t > 2.167, P < 0.05), and they were more in the combination group than in the rope group (P < 0.05), and in the rope group than in the control group (P < 0.05). ConclusionThe combination of PNF rope training and rTMS is more effective on motor function of upper limbs of stroke patients with hemiplegia than PNF training alone.

ABSTRACT

Objective:To explore the optimization of tritium internal exposure monitoring and dose estimation for radiation workers at pressurized heavy water reactor nuclear power plants (PHWR NPPs).Methods:A total of 92 radiation workers at Candu-6 PHWR NPPs were observed to calculate the distribution of quenching index (tSIE). Ten samples with tSIE values ranging from 160 to 460 were selected to make the blank urine quenching curve. By comparing with the quenching curve accompined with the equipment, the optimization of correlationship of urine tritium measurement system was carried out. In estimating tritium internal radiation dose, the dose coefficient was optimized and adjusted by comparing the Canadian algorithm with the national standard algorithm. Dead time correction curve was drawn by means of heavy water simulation experiment diluting main heat transfer system.Results:Through the analysis and optimization of tritium internal dose estimation, the optimal dose coefficient was determined to be 4.8 and the quenching curve of blank urine sample was made. By comparison, it was found that the deviation was larger when tSIE<200, which confirmed the necessity of blank curve. When the tritium concentration in urine was higher than 2.4×10 4 Bq/ml, the urine sample needed to be diluted and then measured to correct the effect of death time. Conclusions:By optimizing both tritium internal exposure monitoring and dose estimation, not only the collective dose management can be improved, but also the unplanned tritium intake can be detected and treated in time, which plays an important role in ensuring the protection and safety of the radiation workers at PHWR NPPs.

ABSTRACT

Objective To investigate the epidemiological characteristics and risk factors of tuberculosis in Cangzhou from 2010 to 2020. Methods A total of 325 patients diagnosed with pulmonary tuberculosis in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2010 to December 2020 were selected as the study group. At the same time, 348 healthy adults who had a physical examination in the same hospital were selected as the control group. The epidemiological characteristics of 325 patients were analyzed. The age, gender, occupation, annual family income, marital status, drinking history, smoking history, education level and tuberculosis contact history of the two groups were compared to analyze the risk factors of tuberculosis. Results The epidemiological and statistical analyses found that among 325 cases of tuberculosis patients, patients ≥60 years old accounted for a relatively high proportion (71.08%). The majority of patients were male by gender (58.15%), and the majority of patients were farmers by occupation (47.69%). The incidence seasons were mainly in winter (40.62%) and spring (29.85%). The results of univariate analysis showed that there were statistically significant differences in gender, occupation, age, history of exposure to tuberculosis, and history of smoking between the two groups (P<0.05). Multivariate analysis found that ages ≥60 years old, male, history of smoking, farmers, and history of exposure to tuberculosis were risk factors for the onset of tuberculosis (P<0.05). Conclusion In Cangzhou, people aged over 60 years, male and farmers are prone to contract tuberculosis, and the peak time of the disease is in spring and winter. The development of corresponding prevention and control measures based on these characteristics is of great significance for the prevention and control of tuberculosis.

ABSTRACT

Clinical microbiology examination technology experiment is an important part of clinical microbiology examination technology teaching. In the experimental teaching of clinical microbiology examination technology, the virtual simulation technology was combined with traditional teaching to give full play to the advantages of the virtual experimental platform. As to experimental projects that couldn't be carried out in traditional teaching and some important experimental projects, students could learn on the virtual experimental platform, and after learning, they would participate in the corresponding assessment. The perfect combination of the two can solve the problem of high experimental cost and limited experimental content in the current experimental class, make up for the shortcomings of traditional teaching, realize the sharing of teaching resources. Besides, it can strengthen the students' experimental operation skills and enhance the interest of learning for cultivation of application-oriented medical talents.

ABSTRACT

Objective To analyze the misdiagnosis reasons and CT features of adrenal gland tumors,and to improve the understanding and diagnosis of this disease.Methods The CT features of 25 cases misdiagnosis of adrenal gland tumors proved by surgically and pathologically were analyzed retrospectively.Results 21 cases were benign tumors including 1 7 cases diagnosed as other benign tumors and 4 cases diagnosed as malignant tumors.4 cases were malignant tumors diagnosed including 3 cases diagnosed as other malignant tumors and 1 case diagnosed benign tumor.Conclusion CT diagnosis of adrenal gland tumors should be closely combined with clinical history,and we should take into account the atypical manifestations of common tumors,as well as consider the possibility of rare adrenal gland tumors,so as to reduce misdiagnosis.

ABSTRACT

Opportunistic bacteria in apical periodontitis (AP) may pose a risk for systemic dissemination. Mucosal-associated invariant T (MAIT) cells are innate-like T cells with a broad and potent antimicrobial activity important for gut mucosal integrity. It was recently shown that MAIT cells are present in the oral mucosal tissue, but the involvement of MAIT cells in AP is unknown. Here, comparison of surgically resected AP and gingival tissues demonstrated that AP tissues express significantly higher levels of Vα7.2-Jα33, Vα7.2-Jα20, Vα7.2-Jα12, Cα and tumour necrosis factor (TNF), interferon (IFN)-γ and interleukin (IL)-17A transcripts, resembling a MAIT cell signature. Moreover, in AP tissues the MR1-restricted MAIT cells positive for MR1-5-OP-RU tetramer staining appeared to be of similar levels as in peripheral blood but consisted mainly of CD4 subset. Unlike gingival tissues, the AP microbiome was quantitatively impacted by factors like fistula and high patient age and had a prominent riboflavin-expressing bacterial feature. When merged in an integrated view, the examined immune and microbiome data in the sparse partial least squares discriminant analysis could identify bacterial relative abundances that negatively correlated with Vα7.2-Jα33, Cα, and IL-17A transcript expressions in AP, implying that MAIT cells could play a role in the local defence at the oral tissue barrier. In conclusion, we describe the presence of MAIT cells at the oral site where translocation of oral microbiota could take place. These findings have implications for understanding the immune sensing of polymicrobial-related oral diseases.

ABSTRACT

Objective To evaluate the relationship between newly developed depression after mild traumatic brain injury (mild-TBI)and abnormality on susceptibility-weighted imaging (SWI).Methods One hundred and eighty patients after mild-TBI,who had normal findings at conventional CT and MRI were enrolled in our study.All cases underwent conventional MRI and SWI.Ac-cording to SWI,the number and volume of microbleed lesions were calculated.All patients were followed up within 1 year after TBI,and divided into depressive group and non-depressive group based on SCID-IV.The difference in microbleed lesions on SWI was compared between the depressive group and non-depressive group.Results The incidence rate of microbleed on SWI in depres-sive group was higher than that in non-depressive group (P 0.05).Conclusion SWI is useful to identify the microbleed lesions after mild TBI.The depres-sion after TBI is correlated with the location and distribution range of microbleed lesions.

ABSTRACT

The incidence of thyroid cancer is the top ranking among endocrine carcinoma worldwide.Many imaging modalities have been applied in diagnosing,characterization of the biological behaviors and predicting the outcomes of various thyroid carcinoma.Over the years,18F-FDG PET/CT has been largely used to identify undifferentiated thyroid carcinoma cells in thyroid carcinoma patients with or without 131I avid lesion.The purpose of this mini-review was to update the clinical role and positive impact of 18F-FDG PET/CT in various thyroid carcinoma patients.
